FAQ
do you take insurance?
We are private pay. While we do not bill insurance directly, if requested, we will provide you with a detailed superbill (receipt) that you can submit to your insurance company for reimbursement. We accept debit cards, credit cards and Health Savings Account Cards.
We are opted out of Medicare. Medicare will not reimburse for our services but if a superbill is requested, it can be sent to your secondary insurance.
What states do you serve?
At this time, we are able to see patients in Colorado but will be expanding to other states soon!
what is telehealth?
Telehealth allows you to discuss symptoms and medical issues with a healthcare provider by phone or video. Using telehealth, you can receive a diagnosis, learn your treatment options, and get a prescription if indicated.
Can I ask questions about my medical issues without an appointment?
We cannot answer any questions about your care until we review your personal health history, listen to your concerns, and ask you about your symptoms. For this reason we recommend making an appointment and bringing a list of your questions to the first visit.
do you prescribe testosterone?
After reviewing your medical history and discussing your concerns, testosterone can be prescribed if indicated. A prescription will be sent to your local pharmacy. If testosterone is prescribed, lab work will be required before and after treatment is started. The prescription will be determined during the follow up appointment.
do you treat erectile dysfunction?
Yes! Sexual health is an important facet of overall health and wellbeing. There are many options available to address sexual health.
What about in person care like lab work, and preventative screening?
It is strongly encouraged that you follow the guidelines on when and if you need routine lab work and screenings. There is a place on the intake form to upload those results. As for lab work in regards to testosterone replacement, it will be ordered after our initial appointment.
Do you provide hormone replacement for women?
Yes we do! Dr. Danielle Shook NP is a menopause society certified practitioner and specializes in treating women in perimenopause and menopause. To book with Dr. Shook, please visit: Summit Hormone
